Aromaterapi, bireyin kendini iyileştirme ve/ya sağlığını sürdürme yeteneğini destekleyen ve güçlendiren bir sağlık sanatı ve bilimidir.
Bu seansın ve/ya gelecek seansların bilgi toplama amaçlı tasarlanmış olduğunu; bu şekilde aromaterapistimin benim eşsiz ihtiyaçlarıma ve sağlıklı yaşam hedeflerime dayalı, aromatik ürünler tasarlayabileceğini ve/ya bu yönde önerilerde bulunabileceğini anlıyorum.
Aromaterapist Rana Babaç Çelebi’nin; herhangi bir rahatsızlığı, hastalığı veya herhangi bir fiziksel veya zihinsel durumu teşhis etmediğini, önlemediğini veya tedavi etmediğini anlıyorum.
Aromaterapist Rana Babaç Çelebi’nin - ister şahsen ister postayla ister telefonla olsun - sağladığı herhangi bir talimat, tavsiye, danışmanlık, öneri, hizmet veya ürünün; herhangi hastalık veya rahatsızlığın tedavi edilmesi, önlenmesi veya hafifletilmesi için değil; sadece vücut sistemlerinin doğal işlevini desteklemek, genel sağlığı ve refahı geliştirmek amacıyla sağlandığını anlıyorum.
Aromaterapi uygulamalarının, ürün ve reçetelerinin; tıbbi tedavilerin yerine geçmediğini ve sahip olduğum herhangi bir fiziksel ve/ya zihinsel durum için nitelikli bir sağlık profesyoneli tarafından muayene edilmem ve tedavi görmem gerektiğini anlıyorum.
Aromaterapist Rana Babaç Çelebi’nin devam eden/oluşabilecek tıbbi bakımları almamamı/durdurmamı tavsiye etmediğini/etmeyeceğini anlıyor ve kabul ediyorum.
Sağlık hizmetim ile ilgili aldığım kararların tek sorumlusu olduğumu ve kararlarımın sonuçlarından Aromaterapist Rana Babaç Çelebi’yi sorumlu tutmayacağımı kabul ediyorum. Aromaterpist Rana Babaç Çelebi’ye, sadece eğitim amaçlı olarak ve kendi özgür irademle danıştığımı anlıyor ve kabul ediyorum.
Yukarıdaki bilgileri okudum, anladım ve bu bilgiler ışığında Aromaterapist Rana Babaç Çelebi’yi; bana, benzersiz ihtiyaç ve hedeflerime uygun bir aromatik program tasarlaması için yetkilendiriyorum.

Aromatherapy is the art and science of welfare that allows the individual to improve his/her health and/or help support and strengthen his/her well-being.
I hereby acknowledge that this and/or the following sessions are designed to collect information; hence allowing my aromatherapist to design and/or recommend aromatic products in accordance with my unique needs and heathy-living objectives.
I understand that Aromatherapist Rana Babaç Çelebi will not diagnose, prevent or treat any preexisting physical or mental condition, disease and/or sickness.
I understand that any instruction, suggestion, recommendation, service or product provided by Aromatherapist Rana Babaç Çelebi - whether in person, on the phone or via mail/email - will not be used to cure, prevent or alleviate any sickness or disease; rather to support the natural function of the body systems and to improve the general health and well-being of the individual.
I understand that aromatherapeutic practices, products or programs are not to replace medical treatments; that any physical and/or mental condition should be attended and treated by a qualified medical professional.
I understand and acknowledge that Aromatherapist Rana Babaç Çelebi does not and will not recommend me to stop/give up on any current or future medical treatment.
I acknowledge that I take the sole responsibility over the decisions I make regarding my health and well-being and that Aromatherapist Rana Babaç Çelebi cannot and will not be held responsible for these decisions and the results thereof. I understand and acknowledge that I consult to Aromatherapist Rana Babaç Çelebi merely for educational purposes and of my own free will.
I read and understood all the information provided above and hereby grant authorization to Aromatherapist Rana Babaç Çelebi to design a personal aromatherapy program for me based on my unique needs and objectives.
